I liked this survey, especially the grid at the end: very inclusive. If this is one that you are giving to your students, ask them how many books they have read this year so far. When I asked my students how often they had read books since January, very few responded with more than 1 (and most because it was required for some class). I think I might steal your survey to use for my classes.
